Key Responsibilities\n

• Conduct initial and ongoing assessments to effectively understand and meet the unique mental health needs of patients.

• Facilitate communication among supervisors, care staff, and family members, ensuring that all parties are informed about the patient's progress.

• Create and maintain schedules for appointments, home visits, and observations to monitor advancements in care.
\n
• Coordinate in-home visits, including observations and data analysis, to gather comprehensive insights into the patient’s care environment.

• Ensure that the implementation of individualized treatment plans is effective and aligns with patient goals.
\n
• Actively participate in interdisciplinary team meetings to discuss strategies for patient care and development.
\n
• Document monthly and quarterly progress notes, accurately reflecting each patient's journey and adjustments to treatment plans.
\n
• Lead training sessions for team members to enhance their skills in implementing treatment plans and completing documentation.
\n
• Provide follow-up support through continued interviews and observations to ensure ongoing patient needs are met.
\n
• Serve as a liaison between the mental health community and the public, fostering collaboration to enhance development goals.

• Assist in the creation of training materials and procedures that enhance the efficacy of our programs.

Patient Population / Client Focus\n

In this role, you will work primarily with diverse populations, including adults and youth, facing various mental health issues such as anxiety, depression, trauma, and substance misuse. Your ability to connect with clients from different backgrounds will be paramount in delivering effective, culturally competent care that resonates with their unique experiences.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Illinois.

• A minimum of two years of professional counseling experience, preferably in a clinical setting, is essential.

• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma will be highly regarded.

• A comprehensive understanding of legal and ethical standards in mental health practice.
