Position Overview

The Pro Active Physio Clinic is currently seeking a skilled and compassionate Physical Therapist (PT) to join our dynamic team in Thousand Oaks, California. Work Environment

This position is primarily onsite; however, it offers flexibility with the opportunity to conduct evaluations and treatments via telehealth when clinically appropriate. You will work primarily within our patients' homes, which requires adaptability and a hands-on approach to care. Key Responsibilities

• Conduct thorough evaluations of each patient's physical capabilities and limitations to determine their unique needs.
• Develop and implement individualized treatment plans that outline specific goals and methods tailored to each patient.
• Monitor and reassess clinical signs and symptoms to evaluate the effectiveness of treatment plans, making adjustments as necessary.
• Complete and maintain all required documentation, including SOAP notes, progress summaries, and care plans, using our electronic record system promptly and efficiently.
• Educate patients and their families on the recovery process, providing them with strategies to overcome physical challenges encountered during rehabilitation.
• Meet established utilization metrics and contribute to the clinic's business objectives while maintaining the highest standards of patient care.
• Collaborate with colleagues across various disciplines at Pro Active Physio Clinic for clients receiving multiple services, ensuring an integrated approach to care.
• Actively promote and exemplify the clinic's core values, serving as a dedicated representative of Pro Active Physio Clinic.

Patient Population / Client Focus

The patients you will serve encompass a diverse population, including children and adults grappling with a wide array of conditions and diagnoses. Your work will directly influence individuals' lives by assisting them in overcoming mobility challenges, recovering from surgery, and managing chronic conditions. Qualifications

• A Bachelor's degree in Physical Therapy is required; a Master's degree or Doctorate in Physical Therapy (DPT) from an accredited program is preferred.
• Must possess a current and unrestricted license from the Board of Physical Therapy to practice in California.
• Proficiency in both in-person and telehealth evaluation and treatment delivery methods.
• Demonstrated passion for working with diverse patient populations, with respect for different ages, races, and ethnicities.
• Ability to adapt to an evolving work environment, demonstrating flexibility and resilience amid changing health care landscapes.
• Strong collaborative skills and a commitment to providing exceptional customer service in every patient interaction.

Preferred Experience

• Experience in home health physical therapy or outpatient therapy settings is highly desired.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience, alongside a comprehensive benefits package.
