Remote Community Health Advocate
Serene Health Group
Optima Medical Management Group is seeking a Community Health Worker (CHW) to serve as a trusted liaison between community members, healthcare providers, and social service organizations. Working as an integral, mostly remote member of the pod's interdisciplinary Care Team, the CHW conducts outreach, provides health education, and connects members to medical and community resources. Under supervision, you will improve health outcomes, promote self-sufficiency, and reduce barriers to care.
