Work Environment\n

This position is remote, allowing the flexibility to work from home while still maintaining a structured schedule. You will primarily engage with clients through virtual platforms to conduct assessments, counseling sessions, and follow-up communications. While you will plan your work hours around client needs, the full-time schedule typically falls between Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours to accommodate urgent client situations. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of senior clients.

• Provide empathetic counseling and psychological support, assisting seniors in successfully navigating life transitions and overcoming personal challenges.

• Work alongside a multidisciplinary team of professionals including healthcare providers, caregivers, and other social workers to coordinate efficient and holistic care.

• Conduct regular virtual visits to monitor clients' wellbeing and living conditions, and address any emerging concerns or safety issues.

• Facilitate effective communication and conflict resolution among family members regarding the care of their senior loved ones.

• Educate clients and their families about available community resources and support programs designed to enhance their engagement and quality of life.

• Document client assessments, service plans, progress notes, and all relevant interactions accurately and promptly.

• Stay updated with current social work practices, compliance regulations, and ethical standards to provide the highest quality of care.

Patient Population / Client Focus\n

The primary focus of this role will be on seniors and their families, who often face significant emotional and psychosocial challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Clinical Social Worker (LCSW).

Compensation and Benefits\n
