Responsibilities
- Coordinate and manage care for inpatients and outpatients across the continuum, aligning plans with patient goals and clinical pathways.
- Assess patient and family needs, facilitate discharge planning, and coordinate necessary referrals and services.
- Develop, monitor, and update individualized care plans with clearly defined goals and expected outcomes.
- Collaborate with physicians, nurses, social workers, and ancillary staff to ensure safe transitions, appropriate resource use, and timely documentation using the electronic medical record.
- Conduct ongoing risk assessments, identify barriers to care, and implement interventions to optimize patient outcomes and utilization.
- Ensure documentation accuracy, compliance with regulatory standards, and adherence to evidence-based practices in case management.

Requirements
- Licensed Registered Nurse (RN) in Colorado; current state license required
- Graduate of an accredited RN program
- Minimum of 3 years RN case management experience
- Hospital case management experience required
- EPIC EMR experience required
- Flu vaccine required with no exemptions
- Ability to work effectively in a hospital setting and collaborate with multidisciplinary teams
