Key Responsibilities

• Conduct thorough mental health assessments of patients, focusing on their emotional wellbeing, coping mechanisms, and potential mental health risks.
• Develop personalized treatment plans that are sensitive to the unique circumstances faced by hospice patients, recognizing their individual needs and goals.
• Offer evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and supportive counseling, to help patients manage anxiety and depression.
• Facilitate both individual and family therapy sessions, enabling patients and their loved ones to discuss concerns, grief, and the emotional challenges that accompany the dying process.
• Regularly collaborate with other healthcare professionals, such as nurses and medical staff, to ensure a seamless approach to patient care and enhance overall care outcomes.
• Participate in case reviews and clinical supervision to continuously enhance your clinical skills and ensure adherence to best practices in therapy.

As a Licensed Clinical Social Worker, you will play a crucial role in addressing these emotional and psychological needs while also supporting families through experiences of grief and loss.

Qualifications

• A Bachelor’s degree in Social Work from an accredited institution; a Master's degree is preferred.
• Current licensure as a Clinical Social Worker (LCSW) in the state of Pennsylvania.
• Strong understanding of mental health issues, particularly in relation to end-of-life care.
• Excellent communication skills, with the ability to convey empathy and support to patients and their families effectively.

Preferred Experience

• Experience in hospice care, grief counseling, or similar healthcare environments.
• Familiarity with interdisciplinary team dynamics and collaborative patient care practices.
• Demonstrated ability to work effectively in a remote setting, utilizing digital communications tools.

Compensation and Benefits
