Work Environment\n

This role will be performed remotely, allowing for flexibility in your work schedule while still contributing to our Detox Center’s operational goals. Regular consultations with interdisciplinary teams, including healthcare professionals and family members, will be conducted virtually, which requires strong proficiency with electronic health record (EHR) systems and digital communication tools.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental health, and social variables.

• Develop personalized care plans in collaboration with clients and their families, ensuring alignment with their specific goals and preferences.
\n
• Provide counseling and emotional support tailored to seniors and their families, assisting them through life transitions and complex challenges.
\n
• Serve as an advocate for clients, connecting them with necessary community resources and ensuring they receive appropriate support.
\n
• Collaborate with an interdisciplinary team of nurses, caregivers, and healthcare professionals to coordinate holistic care for your clients.
\n
• Conduct regular check-ins and site visits (where applicable), monitoring the well-being of clients in their living environments.
\n
• Facilitate family communications and conflict resolution strategies to ensure cohesive support among involved family members.
\n
• Educate clients and their families about community programs, support groups, and additional resources to enhance social engagement.
\n
• Maintain accurate documentation of client interactions, assessments, and care plans in adherence to ethical practice standards.

• Stay updated on current social work trends, policies, and ethical considerations to adhere to best practices.

The focus of this role is on seniors, particularly those facing health challenges, life transitions, or the need for additional support. By promoting a high quality of life through proactive engagement and resources, you will help empower seniors to maintain their independence as much as possible.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
\n
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
\n
• A minimum of two years of experience working in a social work capacity, preferably in settings catering to senior care or community health.
\n
• Solid understanding of social work principles, mental health issues related to aging, and local community resources.
\n
• Exceptional counseling, assessment, and problem-solving abilities.
\n
• Empathy, compassion, and rapport-building skills with clients and families.
\n
• Strong communication skills to effectively collaborate with the healthcare team.

• Experience with EHR systems and comfortability with utilizing various technology platforms.

Compensation and Benefits\n
