Work Environment\n

This role allows for flexibility, enabling you to create your own schedule while working remotely. Our multidisciplinary team utilizes various digital communication tools to ensure effective collaboration and support for one another and our patients. You will have the autonomy to manage your caseload and provide personalized therapy to patients across Fort Collins, Colorado, and beyond.

Key Responsibilities\n

• Perform comprehensive clinical assessments, diagnosing and treating a variety of psychosocial issues that impact patients' lives.

• Utilize evidence-based therapies and treatment modalities, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), tailored to individual patient needs.

• Develop and implement treatment plans in collaboration with the patient, their families, and our integrated healthcare team to enhance treatment adherence and overall patient outcomes.

• Document all social work interventions in compliance with departmental standards while maintaining patient confidentiality and ethical practices outlined by the NASW Code of Ethics.

• Provide education and resources to patients and families to facilitate understanding of mental health conditions and recommended interventions, empowering them in their personal healing journeys.

• Engage in community outreach and patient advocacy efforts, seeking to connect patients with resources that enhance their support systems.

• Participate in regular team meetings to discuss patient cases, share insights, and collaborate on care strategies to ensure the best outcomes for our patients.

• Stay current with relevant community resources, and integrate them effectively into treatment plans to optimize each patient’s care experience.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate school.

We offer a competitive salary in the range of USD 85,000 to 100,000 annually, depending on experience and qualifications.
