Work Environment\n

This role is predominantly remote, allowing for flexibility while maintaining meaningful connections with clients and their families. Our hospice center operates with a multidisciplinary focus, ensuring that our social workers have access to support from healthcare professionals, nurses, and care coordinators to provide holistic care for patients. Key Responsibilities\n

• Conduct thorough evaluations and assessments of patients and their families to identify needs and develop individualized care plans.

• Participate actively in the development and implementation of care plans, including Individualized Family Service Plans (IFSPs) when applicable.

• Engage in ongoing monitoring of outcomes and collaborate with a team of healthcare professionals to adjust care strategies as necessary.
\n
• Facilitate emotional support sessions with patients and families, addressing concerns surrounding grief, loss, and end-of-life decisions.
\n
• Provide education and resources to families about hospice care, coping mechanisms, and available support services.
\n
• Utilize evidence-based practices and culturally competent approaches to build rapport and trust with patients and their families.
\n
• Adhere to all Federal and State regulations as well as internal protocols to ensure compliance and enhance the safety and well-being of all individuals served.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, focusing on providing compassionate support to individuals and families at challenging stages in their lives. You will be responsible for facilitating discussions around treatment options, pain management, and emotional well-being, ensuring that the concerns and wishes of the patients and their families are respected and prioritized.

Qualifications\n

• A Bachelor's Degree in Social Work is required, with a preference for applicants holding a Master's Degree or a Doctorate in Social Work.

• A valid Licensed Clinical Social Worker (LCSW) designation issued by the Department of Public Health is mandatory.

• Excellent communication and organizational skills to effectively work with diverse patient populations and coordinate care effectively.

• A passion for serving individuals and families facing end-of-life challenges with empathy and respect.
