Guaranteed hours: 40 per week
Shift: Evening hours, 3:00 PM to 11:30 PM; Every Other Weekend required
Setting: LTC – Nursing Home

Responsibilities
- Deliver direct patient care and support under the supervision of an RN, focusing on comfort, safety, and daily living activities for long-term residents.
- Administer medications and treatments as prescribed, ensuring proper documentation and adherence to regulatory policies.
- MonitorVital signs, assess patient conditions, and recognize changes; promptly report findings to the supervising RN and care team.
- Implement and update individualized care plans, collaborating with nurses, physicians, and therapy staff to meet resident goals.
- Assist residents with activities of daily living, gait and mobility support, hygiene, nutrition, and skin care; promote dignity and independence.
- Maintain accurate charting and communication, uphold infection prevention protocols, and ensure a safe, clean care environment.

Requirements
- 7-week contract with guaranteed 40 hours per week
- Availability for evening shift (3:00 PM–11:30 PM) and every-other-weekend schedule
- Travel assignment in a long-term care nursing home setting
- Licensure and regulatory requirements applicable to an LPN; ability to meet state practice requirements as posted for North Carolina
