Work Environment

This position will have you working onsite in a home care setting, providing therapy for patients in the comfort of their own homes. You will collaborate closely with a team of healthcare professionals and actively engage with patients' family members, facilitating a supportive environment that promotes recovery and healing. Our team operates under a Monday to Friday schedule, allowing for a healthy work-life balance while delivering exceptional care to our community.

Key Responsibilities

• Conduct thorough evaluations by reviewing patient history and performing comprehensive physical assessments to form appropriate treatment plans.
• Effectively communicate evaluation findings and treatment strategies, engaging both patients and their families in the goal-setting process.
• Administer treatments based on established protocols and adapt as necessary to achieve optimal patient outcomes.
• Document patient progress meticulously in compliance with state regulations and organizational policies, ensuring that all data reflects the patient journey and treatment efficacy.
• Proactively educate patients and their families, equipping them with necessary information on exercises, rehabilitation techniques, and self-care practices to enhance recovery.
• Collaborate with interdisciplinary teams to coordinate care plans and ensure comprehensive treatment approaches.

Qualifications

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 annually, based on experience and qualifications.
