Work Environment\n

This position allows you to work from the comfort of your own home while being an integral part of our Hospice Center team. Although remote, you will collaborate closely with onsite medical teams, including nurses, physicians, and healthcare providers, to provide holistic support to patients. Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand the emotional, social, and economic factors affecting patients’ health.

• Develop and implement personalized treatment plans, offering both individual and group therapy to help patients navigate challenges related to illness, grief, and adjustment.

• Provide crisis intervention and realistic treatment options, ensuring that patients and their loved ones have access to the necessary resources.

Patient Population / Client Focus\n

As an LCSW with Heartstrings & Wellness, you will primarily work with patients in hospice care, along with their families, addressing complex emotional needs and enhancing their quality of life during challenging moments. Your focus will be on fostering coping strategies, understanding grief responses, and providing education about treatment options and resources available.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory, ensuring a solid educational foundation for the role.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice in California.

Compensation and Benefits\n
