Hospice Clinical Manager

Optimal Care

Optimal Care is seeking a Hospice Clinical Manager to lead our hospice team in delivering compassionate, high-quality end-of-life care. In this role, you will oversee day-to-day clinical operations, including staff supervision, patient assessments, and development of individualized care plans. You will coordinate the interdisciplinary team, ensure regulatory and documentation compliance, and drive quality improvement initiatives. The ideal candidate is an experienced RN leader with strong hospice knowledge, excellent communication skills, and a passion for supporting patients and families during critical moments.
