Key Responsibilities

• Provide home health physical therapy services across an assigned territory spanning Culver City south to Long Beach
• Conduct Start of Care (SOC) evaluations and follow-up visits in the patient's home setting
• Meet a daily productivity expectation of 6 points, which may consist of follow-up visits, Start of Care visits, or a combination thereof
• Document patient care accurately and in a timely manner using the HCHB electronic medical records system
• Develop and implement individualized physical therapy treatment plans in accordance with physician orders and patient goals

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Experience with HCHB (Homecare Homebase) EMR system required
• Prior home health physical therapy experience
• Ability to manage an independent caseload across a defined geographic territory

Preferred Experience / Skills

• Familiarity with the Culver City to Long Beach service area
• Strong time management and self-directed work skills suited for a home health environment
