Description
- Thoroughly assess patients in either an inpatient or outpatient context and tailor individualized treatment strategies
- Apply physical therapy modalities focused on enhancing mobility, building strength, improving balance, and restoring function
- Partner with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to achieve exceptional patient outcomes
- Provide education to patients and their families about treatment goals, discharge planning, and at-home exercise routines
- Maintain clear documentation of evaluations, treatments, and progress, adhering to facility protocols and regulatory standards
- Engage in collaborative care rounds and interdisciplinary discussions to ensure seamless transitions of care
Education
- Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution
Certifications
- State licensure required for practice as a Rehabilitation Specialist
- CPR certification preferred
- Additional specialized certifications (e.g., in neurological or geriatric care) are advantageous but not essential
