Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients and their families to identify emotional, social, and environmental concerns related to their current diagnosis or treatment plan.

• Develop and implement individualized treatment plans utilizing evidence-based social work interventions to better support patients during their hospitalization and post-discharge.

• Collaborate closely with a multidisciplinary healthcare team to ensure the involvement of patients and their families in care decisions, fostering a supportive and informed treatment environment.

• Facilitate discharge planning through coordination with various healthcare professionals, third-party payers, and agencies to ensure a seamless transition from the detox center to home or continued care facilities.

• Provide educational resources and counseling to patients and families regarding adaptive strategies to cope with their health conditions and improve emotional well-being.

• Engage in crisis intervention and provide both brief and long-term psychotherapy, employing individual, group, or family therapy as necessary.

• Participate actively in on-call rotations to respond to after-hours inquiries and crises, ensuring that patient needs are met even after standard working hours.

Compensation and Benefits\n

This position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
