SLP Per-Visit Compensation (W-2 Employee): Evaluation / Re-Admission: $100.00 Routine Visit: $80.00 Why SLPs Choose Actify? What You’ll Do: Provide high-quality, one‑on‑one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive‑linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health. Complete Evaluations, Re‑Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge). Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders. What You Need: License: Active Florida Speech‑Language Pathologist (SLP) License.
