Registered Nurse Case Manager

To support beneficiaries in regaining optimum health, the full-time Remote Registered Nurse Case Manager will perform comprehensive assessments, develop care plans, and coordinate care while advocating for self-care management and access to healthcare resources. Key responsibilities Conduct comprehensive assessments to identify clinical, psychosocial, and behavioral concerns and develop individualized care plans Coordinate and implement care plan activities, ensuring continuity of care and effective communication among the healthcare team Monitor and evaluate care plans regularly, revising them as necessary to achieve desired health outcomes Required qualifications Current, unrestricted Registered Nurse license in the state of residence with the ability to obtain multi-state licensure Associate or bachelor's degree in nursing from an accredited institution; bachelor's degree preferred 5+ years of clinical RN experience in direct patient care Proficiency in Microsoft Office and mobile technologies Ability to obtain Security Clearance; current DOD Security Clearance preferred
