We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care.

Role Overview
The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation.

What You’ll Do

• Provide compassionate, patient-centered in-home health evaluations

• Deliver comprehensive assessments, including medication review, medical history, and physical exam

• Engage with patients in their homes, assisted living, and senior community settings

• Build trusted relationships to support better health outcomes

• Travel locally within Los Angeles and Orange County

• Maintain accurate clinical documentation

• Communicate effectively with the care team as needed

Requirements

• Active NP or PA license in California

• Valid driver’s license and reliable transportation

• Experience with geriatric or chronic care populations (preferred but not required)

• Strong communication and interpersonal skills

• Ability to work independently
