As a Nurse Practitioner (NP) or Physician Assistant (PA) with Carease Health - PA, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. Our unique hybrid model combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our goal is to prioritize proactive measures for enhanced physical health and well-being, while also addressing mental health and minimizing hospitalizations.
About the Role:
We are seeking an experienced NP or PA with a background in Geriatrics or Internal Medicine to join our team. As a key member of our clinical team, you will:
Conduct on-site visits to assess patients' physical and mental health
Implement remote patient monitoring (RPM) and chronic care management (CCM) programs to prevent hospitalizations
Collaborate with healthcare providers and facilities to ensure seamless care transitions
Participate in regular training and professional development opportunities to stay up-to-date on industry best practices
Requirements:
Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
