Key Responsibilities:

• Carry out comprehensive annual health evaluations in patients' homes for a Medicare Advantage plan
• Review and assess both current and prior medical history, conditions, and medication use
• Educate individuals about their medical issues and medication regimens
• Detect gaps in care, offering actionable recommendations for improvement
• Focus solely on assessmentno treatment planning or medication prescribing involved

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Master of Science in Nursing from an accredited institution

In this role, compensation is offered at $80-$90 for each completed evaluation, with additional stipends for mileage and administrative tasks. The average assessment is expected to last around 45 minutes, varying by visit type.
