Advance patient care in the community through impactful home health risk assessments as a Nurse Practitioner. This per diem opportunity allows talented clinicians to work directly with Medicare Advantage plan members across Lincoln County, NM, including areas such as Ruidoso, Carrizozo, Capitan, and Ruidoso Downs. Each home visit focuses on comprehensive annual health assessments or post-discharge evaluations, with an average duration of 45 minutes per assessment.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, conditions, and medications
• Educate patients on their health status, chronic conditions, and prescribed medications
• Identify and address potential gaps in care, offering professional recommendations
• Documentation focused; no creation of treatment plans or prescribing required

Qualifications:

• Active Nurse Practitioner License in New Mexico
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Minimum six months recent experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited institution
