Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members in their homes
• Review and evaluate medical history, current conditions, and medication use
• Educate patients on managing their health conditions and proper medication usage
• Identify care gaps and deliver recommendations to help improve patient outcomes
• Focus solely on assessmentno requirement for treatment planning or prescribing medications

Desired Qualifications:

• Active Nurse Practitioner license within NM
• Master of Science in Nursing from an accredited institution
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
