Entry-Level Health & Human Services Case Manager
State of Delaware
State of Delaware is seeking a Health/Human Service Case Manager I to determine initial and ongoing eligibility for needs-based benefits, including SNAP and Medical Assistance, CHIP, childcare, and GA. You will work with SNAP vendors, provide counseling, and refer clients for additional benefits as needed. Responsibilities include developing service plans with hospitals and community resources, documenting case histories, and coordinating care across agencies to expedite services for vulnerable #J-18808-Ljbffr State of Delaware
