Pines Village is looking for a motivated and team-oriented Full-Time Registered Nurse, MDS (Minimum Data Set) Coordinator to join our team. As a member of our care team, the MDS Coordinator will need to work full-time in the office Monday - Friday and be available for the leadership on-call schedule.

Key Responsibilities

• Coordinate, complete, and transmit MDS assessments in accordance with CMS, state, and federal regulations.
• Ensure accurate and timely completion of MDS assessments for Sub-Acute, Medicare, Managed Care, and Long-Term Care residents.
• Demonstrate advanced knowledge of PDPM (Patient-Driven Payment Model) and its impact on reimbursement and resident care planning.
• Utilize PointClickCare (PCC) for MDS documentation, assessment management, care planning, and electronic medical record review.
• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members to ensure accurate resident assessments and documentation.
• Conduct resident reviews and chart audits to verify MDS accuracy, coding compliance, and supporting clinical documentation.
• Participate in and lead the Triple Check Process to ensure billing accuracy and regulatory compliance.
• Develop, update, and oversee individualized comprehensive care plans based on resident assessments, goals, and outcomes.
• Monitor MDS schedules and ensure all assessments are completed and submitted within required timeframes.
• Stay current on CMS regulations, RAI Manual updates, PDPM guidelines, and reimbursement changes.
• Assist with survey preparation and support compliance with quality measures, audit requests, and regulatory reviews.
• Identify opportunities to improve clinical documentation and reimbursement outcomes while supporting resident-centered care.

Qualifications

• Current RN license required.
• Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility (SNF), Rehabilitation, or Long-Term Care setting.
• Proven experience with MDS 3.0, PDPM, Medicare, Medicaid, and Managed Care processes.
• Proficiency with PointClickCare and electronic medical record (EMR) systems.
• Strong knowledge of CMS regulations, RAI guidelines, and federal/state compliance requirements.
• Experience with Triple Check and reimbursement review processes.
• Excellent organizational, analytical, and communication skills.
• Ability to work collaboratively with interdisciplinary teams and manage multiple deadlines.

Preferred Qualifications

• RAC-CT certification or willingness to obtain.
• Experience with Quality Measures (QMs), Five-Star initiatives, and survey readiness.
• Strong background in reimbursement optimization and clinical documentation review.
