Work Environment\n

As a part of our community health clinic, the position offers a fully remote work setup, providing flexibility and convenience in your daily operations. However, you will also be expected to engage in regular site visits to oversee client concerns and ensure the delivery of services tailored to the needs of seniors in various living situations. Our standard work hours are Monday through Friday, 8:00 AM to 4:30 PM, but flexibility in scheduling is paramount to accommodate urgent client needs and ensure continuous care and support after traditional hours.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, integrating emotional, mental, and social dimensions into individualized care plans.

• Work collaboratively with clients and their families to develop tailored care plans that reflect each individual’s goals, preferences, and unique challenges.

• Provide counseling and emotional support to seniors as they confront life transitions, such as medical issues, loss of loved ones, or changes in living situations.

• Serve as an advocate for clients, connecting them with community resources, entitlements, and services necessary for their health and well-being.
\n
• Collaborate with interdisciplinary teams, encompassing nurses, healthcare personnel, and other professionals to foster a comprehensive care approach.
\n
• Conduct home visits to assess clients’ living conditions, provide direct support, and intervene as necessary in safety concerns.
\n
• Facilitate family meetings to mediate any conflicts and enhance communication about client needs and care strategies.
\n
• Educate clients and their families about community programs, resources, and support avenues to encourage social engagement and inclusion.

• Maintain accurate records of assessments, care plans, progress notes, and all relevant client interactions using electronic health record (EHR) systems.

• Stay current on social work practices, emerging policies, and ethical standards relevant to geriatric care.

This role primarily focuses on the senior population, addressing the unique emotional and social challenges faced by older adults. You will work with clients grappling with issues such as loneliness, mental illness, chronic health conditions, and family dynamics, ensuring they receive the holistic support necessary to thrive within the community.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of experience in a social work capacity, ideally within community settings or geriatric care.

• Strong assessment and counseling skills with an ability to create supportive environments for seniors and their families.

Compensation and Benefits\n
