About the RoleThe ADCRR Intake Specialist serves as a key first point of contact for individuals entering New Freedom's ADCRR Transitions Program. This role is responsible for guiding members through the intake and admission process, completing assessments and treatment planning, reviewing program expectations, and ensuring a smooth transition into services. The Intake Specialist partners closely with clinical teams and cross-functional departments to create a positive, supportive, and compliant onboarding experience for every member.

What You’ll Be Responsible For- Conducting intake assessments, screenings, and admission-related activities for new members
- Completing intake documentation, treatment plans, and required program paperwork
- Educating members on program expectations, services, rights, and available resources
- Identifying member needs and ensuring appropriate placement and service coordination
- Facilitating orientations and supporting a successful transition into treatment services
- Collaborating with clinical and cross-functional teams to ensure continuity of care
- Maintaining accurate, timely, and compliant documentation while supporting program performance goals

- Assessments, treatment plans, and intake documentation are accurate and completed on time
- Members clearly understand program expectations, available services, and next steps
- Strong collaboration exists between intake, clinical, and support teams
- Program admissions are completed efficiently while maintaining compliance and quality standards
- Member engagement begins strong, setting the foundation for successful treatment outcomes

What We’re Looking For- Master's degree in Social Work or a related behavioral health field
- Minimum four years of behavioral health experience, including intake assessments and treatment planning
- Strong assessment, interviewing, and motivational interviewing skills
- Excellent communication, organization, critical thinking, and problem-solving abilities
- Ability to work independently and collaboratively within a multidisciplinary team
- Intermediate proficiency with Microsoft Office and electronic health record systems
- CPR/First Aid certification and crisis de-escalation certification
- Must be at least 21 years of age
- Experience working with justice-involved populations preferred
- Knowledge of substance use disorders, co-occurring conditions, and behavioral health treatment services preferred
- Experience conducting biopsychosocial assessments and developing treatment plans preferred
- Familiarity with ADCRR, reentry, or community-based treatment programs preferred
