Work Environment\n

This position is primarily remote, allowing for flexible engagement with clients through virtual consultations as well as in-person visits at designated facilities, including schools and senior living environments. The core working hours are Monday to Friday, from 8:00 AM to 4:30 PM, although some evening or weekend hours may be necessary depending on client needs. This flexibility is vital to ensure that care is continuous and responsive to our seniors' unique situations.

Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify emotional, mental, and social factors affecting their well-being.

• Develop and implement individualized care plans in close collaboration with clients and their families, ensuring that each plan aligns with their preferences and goals.

• Provide ongoing counseling and emotional support, facilitating clients' adaptation to life transitions such as retirement, relocation, or loss of loved ones.

• Advocate for clients’ rights and access to vital community resources, assisting them in navigating complex systems of support.
\n
• Work in partnership with an interdisciplinary team including healthcare professionals, caregivers, and support staff to create cohesive care strategies.

• Perform regular home or facility visits to assess living conditions, monitor clients' status, and address safety or health concerns proactively.
\n
• Facilitate communication among family members involved in the senior client’s care, helping to mediate conflicts and promote collaboration.

• Educate clients and families on available community resources, support groups, and programs that enhance social engagement and well-being.

• Maintain detailed records of assessments, care plans, and progress notes, ensuring compliance with ethical and regulatory standards.

• Stay current with trends in social work practice and developments in legislation affecting the senior demographic.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Current, valid licensure as a Licensed Clinical Social Worker (LCSW-C) in Maryland.

• Knowledge of social work principles, especially as they pertain to aging, as well as a strong understanding of the community resources available.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
