Full-Time Physical Therapist (PT)
We are seeking a full-time physical therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensationwithout on-call requirements.
Key Responsibilities:
- Provide physician-ordered physical therapy services in patients' homes
- Evaluate patient function and develop individualized plans of care
- Complete initial evaluations, reassessments, and plan-of-care updates every 30 days
- Supervise physical therapy assistants and home health aides as appropriate
- Educate patients and caregivers on exercises and home care routines
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation, including OASIS
- Participate in quality initiatives, in-services, and care conferences

Requirements:
- Bachelor's, master's, or doctorate degree in physical therapy
- Active physical therapy license in the state
- Valid driver's license and reliable transportation
- Must be a U.S. citizen or green card holder (no exceptions)
