Facility: Peak Resources Outer Banks
Location: Nags Head, NC
Facility Type: Skilled Nursing Facility (SNF)
Shift: 7:00 PM – 7:00 AM
Schedule: 12-hour shifts
Hours: 36 hours/week
Contract: 13 weeks
Start Date: ASAP
Specialty: LPN – Long-Term Care (LTC)

Job Summary:
Peak Resources Outer Banks is seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate nursing care to residents in a skilled nursing/long-term care setting. The ideal candidate will have strong LTC experience and a history of positive performance on previous assignments.

Responsibilities:

• Provide direct nursing care to residents according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and report changes promptly.
• Perform assessments and document patient care accurately.
• Collaborate with physicians, RNs, CNAs, and other healthcare team members.
• Assist with admission, transfer, and discharge processes as needed.
• Maintain accurate clinical documentation and follow facility policies.
• Provide compassionate care while maintaining resident safety, dignity, and comfort.
• Respond appropriately to changes in residents' conditions and emergencies.
• Follow infection-control, safety, and regulatory standards.

Requirements:

• Active LPN license.
• Previous Long-Term Care/Skilled Nursing Facility experience required.
• Strong clinical and communication skills.
• Ability to work 12-hour night shifts.
• Positive feedback from previous contracts preferred.
• Availability for an ASAP start.

Additional Information:

• On-site housing may be available for approximately $200/week with a $250 deposit .
• Housing is efficiency-style with dormitory-style shower facilities.
• Additional shifts may be available but are not guaranteed.
• Housing and availability should be confirmed before accepting the assignment.
