Work Environment

This position is primarily onsite, as our Physical Therapists work directly within patients' homes. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to assess their physical function and rehabilitation needs.
• Develop, implement, and adjust individualized treatment plans that align with patient goals and physician directives.
• Utilize therapeutic modalities and techniques to promote healing and restore function, ensuring that treatment modalities are clinically appropriate and effective.
• Foster strong therapeutic relationships with patients through consistent communication, support, and empathy, aimed at exceeding their functional goals.
• Educate patients and their caregivers on exercise programs, activity modifications, and use of medical equipment essential for recovery.
• Meet and strive to exceed minimum utilization targets and contribute to the overall performance of the department.
• Participate in continuous professional development and collaborate with a multidisciplinary team to enhance patient outcomes.

Qualifications

• Bachelor's degree in Physical Therapy from an APTA or CAPTE accredited program.
• State License in Physical Therapy, ensuring compliance with local regulations.
• Basic Life Support Certification from the American Heart Association.
• Excellent written and verbal communication skills effective for patient interaction and documentation.
• Proficiency in computer skills and word processing to maintain accurate patient records and treatment documentation.
• A commitment to building strong, therapeutic relationships with patients and their families.
• A desire to positively connect with individuals and contribute to community engagement.

Preferred Experience

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Two years of experience in clinical physical therapy practice, particularly in a home care setting.
• Current CPR certification.
• Experience utilizing modern therapeutic tools and technologies to support patient care.

Compensation and Benefits

We offer a competitive salary ranging between $100,000 and $120,000 annually, complemented by a comprehensive benefits package.
