Intake Coordinator

Ascend Hospice & Palliative Care MA - Now Hiring - Intake Coordinator The Intake Coordinator is responsible for, but not limited to: Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator Insures that whenever possible, all referrals are converted to admissions If referrals do not convert to admissions, the Administrator will be notified Track all inquiries and referrals daily.
