Work Environment

The role is stationed onsite in the homes of our patients, requiring Physical Therapists to perform assessments, develop treatment plans, and deliver care in a variety of home environments. Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, performing specialized assessments, and documenting findings to inform treatment plans.
• Educate patients and their families about evaluation results and collaboratively set realistic and achievable goals for therapy.
• Implement targeted treatment protocols that respond to patient evaluations, adapting strategies as necessary to meet their evolving health status.
• Engage in consultations with other healthcare professionals when required, documenting any changes in patient conditions that necessitate modifications to care plans.
• Facilitate efficient discharge planning and effective follow-up care, ensuring continuity of support for patients transitioning from therapy.
• Document care activities systematically and accurately, including evaluations, treatment plans, and progress notes, to maintain compliance and quality assurance.
• Meet productivity and time management expectations while maintaining an accurate schedule and participating in community outreach initiatives.
• Contribute to departmental goals by collaborating on projects and compliance with healthcare regulations.
• Actively pursue continuing education opportunities to enhance personal competency and contribute to organizational excellence.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, including the elderly, post-surgical individuals, and those with chronic conditions requiring rehabilitation. You will be instrumental in improving patient outcomes through hands-on therapy that emphasizes movement, strength building, and mobility enhancement in the comfort of their homes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure to practice Physical Therapy in Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• While prior experience is preferred, it is not mandatory; we welcome new graduates to apply.

Preferred Experience
