This position is responsible for responding to patient and caregiver needs, providing telephone triage, and making in-person visits as needed throughout the service area.ResponsibilitiesProvide hospice nursing support during the weekend on-call periodRespond to patient and caregiver calls and assess changes in conditionMake in-person visits as needed for symptom management and patient needsProvide end-of-life care and support to patients and familiesComplete hospice admissions, visits, and death visits as neededCollaborate with physicians and the interdisciplinary teamAddress medication, equipment, and supply needsComplete timely and accurate documentation in the EMRCommunicate important patient updates to the weekday hospice teamWeekend AvailabilityMust be available for the full weekend on-call period, Friday at 5:00 PM through Monday at 8:30 AM, including availability for telephone triage and in-person patient visits as needed.QualificationsCurrent Illinois RN licenseHospice or home health experience preferredStrong clinical assessment and communication skillsAbility to work independently and respond appropriately to urgent patient needsValid driver’s license, reliable transportation, and current auto insuranceCurrent CPR/BLS certificationAbility to travel throughout the assigned service area #J-18808-Ljbffr FIRST HEALTH HOSPICE LLC
