Details 25-bed critical access hospital setting Physical Therapy unit providing outpatient and inpatient services Day shift schedule, Wednesday through Sunday, 5x8s from 8:30am to 5pm No call requirements EPIC EMR experience required Requirements Doctor of Physical Therapy (DPT) degree required if licensed on or after January 2016, or foreign equivalent Minimum 2 years of experience in outpatient and inpatient physical therapy required Current Physical Therapy license in state of practice required BLS certification from an approved vendor required License required if applicable Additional Information Responsibilities include conducting evaluations, developing individualized treatment plans, and providing therapy interventions Serves as co-leader in an interdisciplinary approach to patient care Establishes and communicates individualized care and discharge plans Completes accurate clinical documentation within established deadlines Supervises assigned physical therapy assistants Required vaccinations must be maintained annually as a condition of employment, with medical and religious exemptions available No more than 5 days of requested time off (RTO) will be considered No radius rule applies, and local candidates are accepted at the same rate as travel candidates American Traveler Job ID P-748345.
