Nurse Practitioner (NP) - Endocrinology & Type 2 Diabetes Salary: $125,000 - $185,000 (calculated per completed visit, see How compensation works) Hybrid Telehealth | Full-Time or Part-Time | Astoria, Queens About Rockwell Health Center is expanding our endocrinology program and hiring a Nurse Practitioner (NP) to own Type 2 Diabetes, thyroid, and metabolic care for patients across New York. Every visit is delivered by telehealth from our dedicated Astoria workspace, where you are based for most of your scheduled days, with the balance remote. You will carry your own panel with no on-site physician, backed all day by remote support from our endocrinology clinical director and supervising endocrinologist, plus structured education to deepen your specialty knowledge. Full-time and part-time schedules available. Key Responsibilities Manage Type 2 Diabetes, thyroid, and metabolic conditions: assessment, treatment planning, ongoing care Initiate and adjust pharmacologic therapy (insulin, Glucagon-Like Peptide-1 (GLP-1) receptor agonists, oral agents) Order and interpret labs (A1C, TSH, lipids, renal function) and adjust treatment Identify and proactively manage diabetes-related complications, including neuropathy, retinopathy, nephropathy, and cardiovascular risk Counsel patients on nutrition, physical activity, weight management, and medication adherence Use telehealth and CGM (continuous glucose monitor) data for monitoring and follow-up Document care in eClinicalWorks (eCW) and support quality measures including A1C control and HEDIS (Healthcare Effectiveness Data and Information Set) metrics Requirements Master's or Doctoral degree from an accredited Nurse Practitioner program 3,600 or more supervised practice hours, meeting New York's threshold for independent practice Board certification (FNP, AGNP, or ANP) Direct outpatient endocrinology experience required Ability to reliably commute to our Astoria workspace for scheduled in-office days DEA (Drug Enforcement Administration) registration and active malpractice insurance Active enrollment with Medicare (PECOS) and New York Medicaid (eMedNY) as a billing provider; not excluded or barred from Medicare or Medicaid billing Preferred skills CGM data interpretation and insulin pump therapy HEDIS or value-based care familiarity How compensation works Pay is per completed visit: $35.00 per visit for the first 20 visits each day, and $40.00 per visit after that. The annual range reflects expected earnings at typical visit volumes.
