A-Line Staffing is now hiring a Field Case Manager – Long Term Services & Supports (LTSS)
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Pay Rate : - $34.13 an hour
- Benefits – active after 90 days
- Travel : 50–75% local travel; reliable transportation required in Peoria County, IL
- Remote/Field Visits - no office to report to
Key Responsibilities
- Conduct comprehensive evaluations of members' needs, eligibility, benefits, and available resources.
- Identify high-risk factors and service needs that may affect member outcomes.
- Develop, coordinate, and implement assigned care plan activities.
- Monitor member progress and adjust interventions as appropriate.
- Coordinate with case managers, supervisors, Medical Directors, and other healthcare programs to address barriers to care.
- Participate in case conferences and multidisciplinary reviews when needed.
- Identify and escalate quality-of-care concerns through established processes.
- Advocate for appropriate services and resources based on member benefits and healthcare needs.
- Use motivational interviewing and influencing skills to encourage member engagement and healthy lifestyle changes.
- Provide members with coaching, education, information, and support to promote independent healthcare decision-making.
- Help members actively participate in healthcare decisions with their providers.
- Maintain accurate and timely documentation in accordance with regulatory, accreditation, and company requirements.
Required Qualifications
- Bachelor's degree in a human services-related field
- Minimum of 2 years of case management experience
- Must reside in Peoria County, Illinois
- Valid Illinois driver's license
