Work Environment

The position is based in a home care setting, which allows for a unique opportunity to interact with patients in the comfort of their own homes. You will work Monday through Friday, from 8:00 AM to 5:00 PM, ensuring a balanced work-life experience. Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing medical histories and performing specific assessments to inform treatment recommendations.

• Discuss evaluation findings and treatment plans with patients and their families, encouraging participation in goal setting to foster a therapeutic environment.

• Implement treatment protocols effectively, adapting approaches based on evaluation outcomes and patient feedback to optimize rehabilitation efforts.

• Consult with healthcare professionals to ensure holistic patient care, documenting any changes in patient conditions that may require adjustments to treatment plans.

• Manage discharge planning and follow-up care, ensuring all documentation is accurate, timely, and reflective of the patient's continuum of care.

• Exhibit strong time management skills, adhere to productivity targets, and maintain an organized patient schedule to enhance service efficiency.

• Contribute to departmental goals and compliance with regulatory standards, ensuring exceptional care delivery at all times.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.

• A valid state license to practice physical therapy as required in New Jersey.

• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

• A commitment to ongoing professional development and lifelong learning in the field of physical therapy.

Preferred Experience

A minimum of two years of relevant clinical experience is preferred, particularly in home care or outpatient settings. Familiarity with evidence-based treatment practices and experience with diverse patient populations will be advantageous in this role.

Compensation and Benefits
