Key Responsibilities

• Conduct comprehensive evaluations of patients, including gathering pertinent medical history and performing specific physical assessments to establish individualized treatment plans.
• Effectively communicate evaluation findings and treatment goals to patients and their families, ensuring their involvement in the rehabilitation process.
• Utilize evidence-based practices to implement treatment protocols, adjusting them as needed based on patient progress and responses.
• Collaborate with multidisciplinary teams, including physicians and therapists, to coordinate optimal patient care and modify plans as necessary based on observed health changes.
• Document patient progress meticulously, completing notes for evaluations, treatment sessions, and discharge planning within agreed-upon timeframes.
• Meet productivity expectations while managing your schedule efficiently and participating in marketing initiatives that promote access to care.
• Contribute to departmental objectives and compliance with governmental regulations while focusing on patient-centered care.
• Engage in ongoing professional development, serving as a mentor and clinical instructor for students and new staff.
• Encourage a positive patient experience through adaptability and dedication to access health services.
• Assist in identifying and addressing barriers to care, actively participating in quality improvement initiatives.

Patient Population / Client Focus

Our patient population includes individuals recovering from surgeries, injuries, and chronic conditions, as well as elderly patients requiring rehabilitation to enhance their quality of life. The Home Care setting allows us to provide personalized attention, working with patients in their own environments, which significantly enhances the therapeutic process and overall outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A relevant and valid state license to practice physical therapy in New Jersey.
• Current American Heart Association's BLS for Healthcare Providers certification.
• Knowledge of home care services and experience with diverse patient populations is highly desirable.

Preferred Experience

While a minimum of two years of experience as a Physical Therapist is required, we welcome new graduates who are eager to learn and demonstrate a commitment to ongoing personal and professional development. We value continuous education and growth opportunities within our team.

Compensation and Benefits
