Work Environment

This full-time role is strictly onsite, as you will provide therapy services directly within our patients' homes. Key Responsibilities

• Conduct initial patient evaluations to assess needs and develop tailored rehabilitation plans that align with individual goals and capabilities.
• Implement treatment protocols dynamically based on patient evaluations and evolving health needs, using techniques such as manual therapy, exercise prescriptions, and modalities.
• Document patient progress meticulously, including baseline assessments, treatment responses, and follow-up evaluations, to ensure compliance with regulatory standards and facilitate clear communication with the healthcare team.
• Lead discharge planning processes, setting patients up for success after formal therapy ends by recommending ongoing activities or referrals to additional services as needed.
• Regularly communicate with physicians, caregivers, and other healthcare professionals to ensure comprehensive care coordination.
• Maintain a clean and safe working environment in each patient's home, utilizing effective methods to manage and ensure compliance with health regulations and best practices in home care.

Patient Population / Client Focus

As a Physical Therapist at Headstrong Rehabilitation & Wellness, you will primarily focus on patients recovering from orthopedic procedures, accidental injuries, and chronic pain conditions. Our clientele is diverse, spanning various ages and backgrounds, each with unique health challenges and rehabilitation needs. Your expertise will be pivotal in guiding them through recovery, helping to restore mobility and enhance their overall well-being.

Qualifications

• A Bachelor's Degree in Physical Therapy (DPT preferred) and a valid license to practice as a Physical Therapist in New Jersey.
• Certification in CPR through the American Heart Association is required.
• Strong communication skills and the ability to foster rapport with patients and their families, ensuring a supportive rehabilitation process.

Preferred Experience

While a minimum of 2 years of relevant experience in a clinical or home care setting is required, candidates with additional experience in orthopedic therapy or specialized certification in areas such as sports therapy or geriatrics will be considered favorably. Proficiency in electronic health record (EHR) systems is also preferred to enhance documentation efficiency.

Compensation and Benefits
