Key Responsibilities

• Conduct thorough evaluations by examining patient history, performing specific physical assessments, and documenting findings to develop personalized treatment plans.
• Discuss assessment results and proposed treatment plans with patients and their families, ensuring their involvement in setting realistic rehabilitation goals.
• Implement evidence-based treatment protocols that respond dynamically to each patient's unique circumstances to optimize recovery outcomes.
• Collaborate effectively with other healthcare professionals, recognizing and documenting any changes in patient status that necessitate adjustments in care strategies.
• Manage discharge planning comprehensively, ensuring that follow-up care aligns with the established treatment goals, while meticulously documenting each phase of patient care.
• Uphold productivity standards by efficiently managing time and scheduling commitments, attending team meetings, and participating in marketing initiatives to promote community health awareness.
• Contribute to departmental objectives and comply with all relevant licensing and accreditation standards to uphold the highest quality of care.
• Engage in continuous professional development activities, including serving as a clinical instructor for students, fostering the next generation of healthcare providers.
• Enhance patient access to care through flexibility and innovative solutions that address systemic barriers in healthcare delivery.

Patient Population / Client Focus

This position primarily involves working with a diverse patient population within their home environment, accommodating individuals recovering from surgery, injury, or chronic conditions. Each treatment plan is tailored to enhance functional independence and improve quality of life, delivering care that addresses the unique needs and preferences of each patient.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Possession of a valid state license as a Doctor of Physical Therapy.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• State-specific licensure as required for your practice location.

Preferred Experience

• A minimum of 2 years of clinical experience as a Physical Therapist is preferred.
• Experience in home health or outpatient settings is a plus.

Compensation and Benefits
