Work Environment

This position is based onsite and involves working in patients' homes throughout the Middletown area. Key Responsibilities

• Conduct thorough evaluations of patients to identify their physical therapy needs and establish personalized treatment plans aimed at promoting recovery and mobility.
• Manage an efficient workload typically consisting of one patient per hour, ensuring high-quality care is provided within that timeframe.
• Utilize advanced techniques in manual therapy and various therapeutic modalities to enhance patient outcomes.
• Collaborate closely with an interdisciplinary team of healthcare professionals, including physicians and occupational therapists, to optimize patient care plans and ensure holistic treatment approaches.
• Monitor the progress of patients, documenting changes meticulously and adjusting treatment plans as necessary to achieve the best results.
• Maintain accurate and up-to-date documentation of patient evaluations, treatment plans, progress notes, and billing information in compliance with regulatory standards.

Patient Population / Client Focus

You will primarily serve patients requiring rehabilitation due to orthopedic conditions, post-operative recovery, and chronic pain. Assessments and treatments will be tailored to each patient, factoring in their unique histories and goals, which highlights the importance of personalized care.

Qualifications

• A Doctorate in Physical Therapy (DPT) from a CAPTE-accredited program.
• A valid New Jersey license to practice as a Physical Therapist.
• A clean professional record with no disciplinary action or board actions against the license.
• Expertise in manual therapy techniques and a strong foundation in orthopedic rehabilitation practices.
• Ability to operate both independently and collaboratively, demonstrating effective communication skills.

Preferred Experience

The ideal candidate will have a minimum of two years of relevant professional experience in providing physical therapy services, specifically in outpatient orthopedic settings. Familiarity with home care practices will be advantageous.
