Full-Time Licensed Physical Therapist for Home Care Services

Desert Blossom Orthopedic and Sports is seeking a dedicated and compassionate Physical Therapist (PT) to join our team on a full-time basis. This position entails providing high-quality home care therapy that focuses on enhancing client quality of life through personalized treatment plans. Work Environment

This position is based onsite, primarily in client homes across Keyport, New Jersey. As a part of our home care facility, you will work directly within diverse home environments, tailoring therapeutic interventions to each unique setting. This hands-on role requires adaptability and creativity, ensuring our clients receive the highest level of care in the comfort of their own homes.

Key Responsibilities

• Develop and implement tailored physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring maximum functionality in accordance with prescribed plans of care.
• Oversee the work and activities of Physical Therapy Assistants to ensure the effective execution of treatment plans and adherence to best practices.
• Conduct thorough evaluations using diagnostic tests to assess muscle, nerve, joint, and functional abilities, enabling accurate and customized treatment approaches.
• Assess the home environment to identify factors affecting the client's rehabilitation and recommend modifications to enhance independence.
• Deliver prescribed physical therapy interventions, including therapeutic exercises, traction, massage, and the application of physical agents.
• Evaluate, fit, and adjust prosthetic and orthotic devices as needed, communicating recommendations effectively to physicians.
• Contribute actively to interdisciplinary team case conferences, collaborating with other healthcare providers for holistic treatment planning.
• Maintain accurate clinical records, submitting initial evaluations, care plans, progress notes, and discharge summaries in compliance with agency protocols.
• Guide clients and their families in understanding and implementing their physical therapy programs, enhancing their engagement in the recovery process.
• Complete functional assessments regularly to ensure the ongoing relevance of treatment plans and utilize community resources optimally.

Patient Population / Client Focus

Your work as a Physical Therapist will focus on a diverse patient population, including individuals recovering from surgery, sports injuries, chronic pain conditions, and age-related functional decline. You will be responsible for creating a supportive and empowering environment that fosters patient adherence to rehabilitation goals and encourages active participation in their care.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• An active Physical Therapist license in the state of New Jersey is essential, with a clean record devoid of sanctions or board actions.
• Exceptional skills in manual therapy modalities and a proven ability to design effective exercise programs are mandatory.
• Competence in electronic medical record (EMR) software, particularly Web PT, for efficient documentation and follow-up is necessary.
• A solid understanding of contemporary physical therapy techniques, backed by evidenced practices, is essential.

Preferred Experience

• Prior experience in home health care settings is a significant advantage, as it emphasizes flexibility and adaptability.
• Experience collaborating with interdisciplinary teams to streamline patient care pathways and improve treatment outcomes enhances your candidacy.
• Knowledge of insurance processes related to home care therapy can be beneficial in navigating patient care logistics.

Compensation and Benefits

We offer a competitive salary that ranges from USD 100,000 to 120,000 annually, complemented by a comprehensive benefits package.
