This full-time, onsite position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience. Work Environment

As a Physical Therapist working in the home care setting, you will engage directly with clients in their own environments, enabling you to evaluate and adapt treatments that best suit their lifestyles. This participatory setting fosters a collaborative approach, not only involving clients and their families but also integrating feedback from interdisciplinary team members to optimize therapy outcomes. Your daily responsibilities will include working from Monday to Friday, allowing for a balanced professional lifestyle.

Key Responsibilities

• Implement and direct personalized physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and restoring optimal muscle function while accommodating each client's capabilities.
• Oversee the activities of Physical Therapy Assistants, ensuring adherence to the therapy plans and enhancing treatment delivery.
• Conduct thorough evaluations of clients' physical capabilities by administering tests that assess muscle, nerve, joint functionality and overall mobility, adjusting care plans based on findings.
• Assess home environments to make informed recommendations that promote client independence and optimal function.
• Deliver prescribed physical therapy services at clients' homes, adhering strictly to physician guidelines.
• Guide clients through manual therapeutic exercises and provide motivation to encourage adherence to prescribed non-manual exercises and functional activities.
• Administer various treatment modalities, including traction and massage, while utilizing physical agents to support recovery efforts.
• Evaluate and adjust the fitting of prosthetic and orthotic devices, collaborating with prescribing physicians on needed modifications.
• Contribute to interdisciplinary team meetings, providing insights on client progress, care plan effectiveness, and collaborating with other healthcare professionals as required.
• Maintain accurate clinical documentation, encompassing initial evaluations, progress notes, and patient discharge summaries to ensure cohesive client records.
• Educate clients and their families about rehabilitative methods, including the correct usage of therapeutic devices and comprehensive therapy programs.
• Complete additional administrative tasks, such as submitting clinical documentation and payroll records, in accordance with branch policies.
• Engage in continuous professional development to stay updated on the latest physical therapy techniques and best practices.

Patient Population / Client Focus

The clients you will serve include a diverse population requiring personalized rehabilitation plans due to injuries, surgeries, chronic conditions, and age-related ailments. Your ability to adapt therapy to their respective environments and individuality will play a significant role in their treatment journey and overall recovery outcomes.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program.
• Possession of a valid Physical Therapist license in the state of New Jersey, with no history of sanctions or disciplinary actions.
• Proficiency in employing a variety of manual therapy techniques.
• Skilled in developing effective exercise programs tailored to individual client needs.
• Experience using EMR software, specifically WebPT, to document client progress proficiently.
• Strong knowledge of evidence-based physical therapy practices and current trends in rehabilitation.

Preferred Experience

• Experience in home health care settings is advantageous and preferred.
• Previous supervisory experience of Physical Therapy Assistants is a plus.
• Familiarity with therapeutic modalities and the ability to adapt treatments to evolving client needs.
