Care Point Physical Therapy is seeking a dedicated and skilled Orthopedic Physical Therapist to join our team in Lincroft, New Jersey. This full-time position is focused on delivering exceptional therapy services directly in clients' homes, fostering rehabilitation and enhancing their quality of life. Work Environment

This position is fully onsite, working within clients' homes in the Lincroft area. You will collaborate with a comprehensive interdisciplinary team to ensure cohesive therapy and rehabilitation. Key Responsibilities

• Develop and direct individualized physical therapy treatment plans aimed at reducing pain, preventing disabilities, and enhancing muscle function, aligning with the prescribed care strategies.
• Oversee and guide Physical Therapy Assistants in executing treatment plans effectively, ensuring compliance with legal and ethical standards.
• Conduct comprehensive evaluations of each client's functional abilities through specialized diagnostic tests and assessments that determine mobility, strength, and overall physical health.
• Assess and modify the home environment as necessary to promote independent living and facilitate optimal rehabilitation outcomes.
• Implement a variety of physical therapy interventions, such as manual therapeutic exercises, administration of modalities (traction, massage), and patient education in home exercise regimens.
• Create and maintain accurate clinical records, including evaluations, care plans, and progress notes, to document treatment and inform ongoing therapy strategies.
• Actively participate in interdisciplinary care plan meetings, offering insights that contribute to the holistic treatment goals for clients.
• Provide education to clients and their families about therapy devices, facilitating proper use and adherence to rehabilitation programs.
• Regularly communicate updates regarding client conditions, treatment responses, and recommended adjustments to care plans with physicians and case managers.
• Supervise Physical Therapy Assistants during their hands-on therapy sessions to ensure best practices are being followed according to applicable state laws.
• Complete additional assignments as directed to meet organizational goals and enhance patient outcomes.

Patient Population / Client Focus

This role focuses on a diverse clientele with orthopedic concerns, including individuals recovering from surgery, injury rehabilitation, and those managing chronic pain conditions. Qualifications

• A graduate of an accredited physical therapy program, demonstrating strong foundational knowledge and skills.
• At least one year of professional experience as a physical therapist, preferably in an acute care or home care setting.
• Current state licensure as a Physical Therapist, with good standing.
• Certification in CPR and current liability insurance.
• A valid driver's license along with automobile insurance for travel to client locations.
• Demonstrated ability to think creatively and independently, managing complex decisions with confidence.

Preferred Experience

• Experience specializing in orthopedic rehabilitation is highly desirable, enhancing your effectiveness with our patient demographic.
• Previous home care experience will equip you with the unique skills required to navigate client care in their own environments.

Compensation and Benefits

Care Point Physical Therapy offers a competitive salary range between USD 100,000 and 120,000 yearly, commensurate with experience and qualifications.
