This full-time position offers an annual salary ranging from USD 100,000 to 120,000, depending on experience. Work Environment

This position is based onsite, involving direct client interactions in the comfort of their homes. As part of our interdisciplinary team, you will collaborate closely with other healthcare professionals to ensure comprehensive care and support for our diverse clientele. Our home care setting allows you to build meaningful relationships with clients, understand their unique environments, and tailor therapeutic interventions accordingly.

Key Responsibilities

• Direct and implement physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle functionality according to each client's individualized needs.
• Evaluate clients' physical abilities through diagnostic tests, assessing their progress and adapting treatment plans as necessary to optimize recovery outcomes.
• Work collaboratively with Physical Therapy Assistants, overseeing their practice to ensure quality care and adherence to specific treatment protocols.
• Conduct on-site environmental assessments to identify opportunities that can increase client independence and enhance their rehabilitation experience.
• Administer a variety of therapeutic techniques, including manual exercises, traction, and massage, while guiding clients through non-manual exercises and functional activities.
• Adapt prosthetic and orthotic devices to meet client needs and provide recommendations to medical personnel for adjustments.
• Maintain accurate clinical documentation, including initial evaluations and progress notes, ensuring compliance with OASIS standards and submission deadlines.
• Actively participate in interdisciplinary team discussions, providing insights into client progress and collaborating on comprehensive care strategies.
• Educate clients and their families on effective use and care of therapy equipment, as well as overall treatment methodologies.
• Complete all required administrative tasks, including timely documentation and payroll records, in accordance with company policies.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population, delivering care primarily to individuals recovering from surgeries, injuries, and medical conditions that impact mobility and functional skills. Your insight into each client's home environment will allow you to create therapeutic strategies that are feasible and impactful, facilitating a seamless transition from clinical settings to home-based rehabilitation.

Qualifications

• A Bachelor's Degree in Physical Therapy or, preferably, a Doctorate in Physical Therapy (DPT).
• Current and valid Physical Therapy licensure in New Jersey, maintaining good standing throughout employment.
• One (1) year of relevant experience in physical therapy, particularly in home care settings.
• Certification in CPR to ensure patient safety and emergency preparedness.
• A strong commitment to clinical excellence and integrity, with a focus on delivering quality patient care.
• Effective communication skills, capable of engaging with both patients and their families to foster collaborative treatment environments.
• A positive, proactive attitude, with the aptitude for teamwork and adaptability in a dynamic healthcare setting.

Preferred Experience

• Prior experience in developing and implementing individualized care plans within a home care framework.
• Demonstrated ability to utilize electronic health records (EHR) effectively for documentation and communication purposes.
