Work Environment

This role is full-time and primarily involves onsite work in patients' homes. The flexibility of setting appointments allows for a manageable work-life balance within a supportive team atmosphere. As part of our home care service, you will have the opportunity to work one-on-one with your patients, developing personal rapport and tailoring treatments to their unique circumstances. Key Responsibilities

• Conduct comprehensive evaluations of patients to identify their physical therapy needs, assessing factors such as mobility, strength, and pain levels.
• Develop and implement individualized treatment plans tailored to each patient, utilizing a variety of therapeutic modalities including exercise, manual therapy, and functional training.
• Regularly monitor and document patient progress, adjusting treatment plans as necessary to reflect changes in patient condition or therapy effectiveness.
• Educate patients and caregivers on their conditions, treatment plans, and the use of any rehabilitation equipment or assistive devices.
• Maintain open lines of communication with other healthcare professionals involved in the patient's care, ensuring a coordinated approach to rehabilitation.
• Stay current with advances in the field through workshops and other educational resources to continuously enhance your skills and knowledge.

Patient Population / Client Focus

The patients you'll work with range from pediatric to geriatric individuals facing a variety of challenges, including post-surgical recovery, sports-related injuries, chronic pain conditions, and neurological disorders. Your empathy and dedication will be crucial in helping these patients reclaim their independence and improve their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is required.
• A valid state license to practice physical therapy in New Jersey.
• Exceptional interpersonal and communication skills, both written and verbal.
• Proficiency in using electronic health record (EHR) systems and other standard healthcare technologies.
• A strong ability to adapt to various circumstances, displaying flexibility in roles and responsibilities based on patient needs.

Preferred Experience

• Experience in home care settings is a plus, but not mandatory; we welcome new graduates eager to learn.
• Prior experience with orthopedic rehabilitation techniques or working with assistive devices.

Compensation and Benefits
