Candidate Pay Rate: $52 per hour

Roles & Responsibilities

• Conduct in-person participant assessments to determine the right level of care
• Deliver hands-on wound care, IV hydration, lab draws, and nursing assessments during home visits
• Respond promptly to calls from providers and support staff, prioritizing needs with care
• Document all interactions thoroughly and follow up as needed to ensure continuity
• Educate participants, families, and facility staff with compassion and clarity
• Collaborate with providers, pharmacies, hospitals, and community partners for seamless support
• Initiate emergency treatment when necessary and keep leaders informed
• Champion quality improvement by completing incident reports and participating in ongoing training

Qualifications

• Active Michigan RN license required
• Associate degree in Nursing required; Bachelor's preferred
• At least one year of experience with frail, elderly, or long-term care populations
• Home care experience preferred
• Wound care experience preferred
• Current CPR certification by AHA or willingness to obtain
• Reliable transportation, valid Michigan driver's license, and car insurance

Education

• Associate degree in Nursing required
• Bachelor's degree in Nursing preferred

• Full-body assessment and critical thinking skills
• EMR documentation proficiency
• Strong communication and collaboration across care teams
• Ability to adapt to changing workloads and schedules
• Participant-centered care approach
