Aging Services Outreach & Referrals Specialist

Sunnyside Community Services

Sunnyside Community Services in New York seeks a Community Outreach Worker to promote the Case Management program among local organizations and health providers. You will build relationships, refer eligible adults to services, and coordinate with partners to expand program reach. The role requires strong communication, bilingual abilities, and experience engaging with aging populations.
