Clinical Care Manager

Job Overview: The Clinical Care Manager is a hybrid full-time role (32-40 hours), with therapy visits and care manager visits split roughly evenly throughout the week. This role is designed for an Associate Clinician whose experience matches both roles: providing psychotherapy while also holding a care management panel. The Clinical Care Manager reports to our Director of Behavioral Health and also has oversight by our Complex Care Team Supervisor. Reflective supervision for pre-licensed BBS hours is included. Key Responsibilities: Clinical Care — approximately 50% In partnership with HH & Co., grow and manage a panel of patients in a clinic setting Practice and develop clinical skills that apply to all populations seeking care and connection Facilitate virtual (video and phone) and in-person sessions with patients Accurately assess, diagnose, and develop treatment plans Maintain accurate and timely clinical documentation Participate actively in group supervision, individual supervision, and team meetings Develop and maintain clinical commitment to cultural resonance and equity standards of the practice Care Management — approximately 50% Provide care management to patients of all ages Manage a care management panel and maintain minimum visit requirement per week Practice Motivational Interviewing and lead with empathy Maintain timely care management documentation practices Support the development and implementation of the Enhanced Care Management (ECM) line of service, including program protocols, referral processes, documentation, and claims submission Collaborate with the broader team with a spirit of generosity and openness Panels and hours build gradually: this role typically begins with a smaller combined caseload across both halves of the work and ramps toward full panel targets over the first two months, consistent with HH & Co.'s approach to holding care as panels build. Minimum Qualifications: Master's Degree in Social Work, Marriage and Family Therapy, Clinical Counseling, or another related field Registered with CA BBS (Associate) or currently licensed, with a current CAQH profile 2+ years of direct client/patient service experience Strong communication skills, both written and verbal, with a focus on clarity, professionalism, and responsiveness Strong emotional intelligence (EQ), with the ability to understand and respond to the needs of both patients and team members with empathy, confidentiality, and professionalism Preferred Qualifications: Experience in integrated behavioral health settings Experience in provision of care for individuals experiencing SUD, SMI, and complex co-occurring medical conditions Interest or experience in macro-level community health initiatives EMDR trained Pay Range: Compensation matches our rate for Associate Clinicians ($38 per hour) for time spent in both roles.
